Eye Vision Prescription Form?

I just finished my CFAT/medical/interview the other day. They gave me a form for my optomatrist to fill out for my eyeglass prescription, unfortunately it's at my mothers house and I can't go get it.
Does anyone know if there's a copy of that form on the internet I can print out? I wanted to get it in as soon as possible and I can't wait for my father to get back into town.

Did you call the CFRC see if they have the electronic version and have them email it to you?

I emailed the recruiter asking about it already, no answer as of yet.
He's usually really quick about answering my emails, but it's still early.

The one's I used to send out were locally produced IIRC, not an official CF form.

The recruiter answered my email, he said only the med tech has access to the form.
I'm going to have to wait or try and get my form, somehow.  =(

Oh well, it's my fault. Shouldn't have left it there in the first place.
