Referencing a webpage in a memo

What would be the format for referencing a website in a memo? I am writing a memo which references guidlines which are set force on a webpage.
 
I would put it like this:

A.  Guidelines for completing a CF 942 tag, FLOG Halifax website (http://www.whateverwebsite.ca/someuselessinfo.htm)
 
There may be more correct formats, but here is an example of a format I have used.  I haven't received any complaints yet!

CIA. "The World Factbook - Canada" Last accessed 15 January 2013.      https://www.cia.gov/library/publications/the-world-factbook/geos/ca.html

I think it's important that not only do you include the internet address, you should also include the date on which you accessed the information as sites may be updated.

However, if your are referencing something like a CFAO, QR&O, CBI  which are mostly electronic now.  You would still just put the pub ref not the web site.  For example:

Refs. A.  CFAO 49-4 para 2
B. CBI 209.43
